Share & Care Community Services Group has two programs where seniors can access support to remain independent at home.
The Commonwealth Home Support Program is an entry-level program to help older people live as independently as possible.
It is about working on a client’s strengths, capacity and goals to help them remain living independently and safely at home.
Seniors may be eligible if they are:
- Having trouble doing everyday activities without help
- Need support to live independently in the community
- Are 65 years or older (or 50 years or older and identify as an Aboriginal or Torres Strait Islander person)

Home care packages are a coordinated package of services to help clients live independently in their own home for as long as they can.
There are four levels of packages to help meet the different levels of care needs.
During an aged care assessment, a member of the aged care assessment team will discuss your current care needs and determine the best level to meet them.
The benefit of a home care package is that a home care provider will work with clients to choose care and services that best meet the client’s needs and goals and work to manage the client’s services.
